WILTSHIRE Council’s Health and Wellbeing Board has created the Joint Health and Wellbeing Strategy to improve quality of life in Wiltshire communities.
The Better Care Plan details how the strategy will be delivered and outlines how a £27m pooled budget will be used and how the board will ensure the money reaches the right areas.
Over the next two years the plan aims to provide care close to home, a joint approach to assessments and care planning, better data sharing between health and social care, and a seven-day service.
Keith Humphries, Wiltshire Council cabinet member for public health, said: “If you receive care from professionals who have all the information about you at their fingertips, this can save time and make your care more efficient. Being cared for in their own homes can be important for so many people and having more services available in the community which mean you don’t have to stay in hospital but can be looked after at home is an important part of the plan.”
“By working together with our health colleagues we want to be able to build on the good work already being achieved in Wiltshire and take it to the next level.”
