The second day of the inquest into the death of Adrian Kowalik, who died after being hit by a train last year, has come to an end. Today, Salisbury Coroner's Court heard from social psychiatry professor Rob Poole who believes Mr Kowalik shouldn't have been allowed to leave Great Western Hospital just hours before his death,.
