THE DEATH of Patrick Bradley was labelled a tragedy by children's charity the NSPCC.

Following the publication of Thursday's serious case review, the organisation called for the review's recommendations to be fully carried out.

A spokeswoman for the NSPCC in the south west of England said: “This is a tragic case where a baby lost its life at just ten weeks old.

“Thursday's report recognises that lessons have been learned about the way agencies communicate and work with each other, including in cases where there are concerns about the welfare of a child before they are born.

“But it’s essential that the recommendations from today’s report are addressed in full.”
