KOWALCZYK Dr. Adam Michael On 4th April aged 68 years. Formerly a family GP at Eldene Surgery, Swindon Much loved partner of Ann and father of Penny and Al. He loved photography, aeroplanes, good wine and cake. Funeral service to take place at Kingsdown Crematorium on 12th May at 12 noon.
